When an exception is thrown, sf console doesn't show the code exception, while this code is very useful during developpement (for example, [guzzle put http status code in his code exception](https://github.com/guzzle/guzzle/blob/6.1.1/src/Exception/RequestException.php#L30-L33)).
I propose you that we show the code exception only when the code exception > 0 and with ``-v`` verbosity. It means :
- ``throw new Exception('Not found', 0)`` with normal verbosity --> no change
- ``throw new Exception('Not found', 0)`` with ``-v`` verbosity --> no change
- ``throw new Exception('Not found', 404)`` with normal verbosity --> no change
- **but** ``throw new Exception('Not found', 404)`` with ``-v`` verbosity --> showing :

The first commit is only about -- support + fixes a few other little issues I discovered while adding tests to cover my feature.
The second commit makes use of the first in the Application class, so that if you do `console foo -- --help` for example, it should not trigger the help anymore, but simply have --help available as a dummy arg.
To keep BC, I did this by adding a param to hasParameterOption/getParameterOption instead of doing it by default. This creates another BC issue though in that it changes the InputInterface. I don't think that's a major concern but it's worth mentioning.
The other only minor BC change is if you do getParameterOption() of a flag option i.e. `--foo` without a value. Before it was returning null in ArgvInput and true in ArrayInput. I normalized this to true for both since I think that makes more sense, but strictly speaking while it improves interface interchangeability, it's a BC break.

This is an alternative to the `ProgressBar` helper *without a max* inspired by the `npm` cli. You can use the `ProgressBar` but IMO it doesn't look great or fallback nicely in non-ansi environments.
A lot of work needs to be done still but thought I would get some comments on this idea.

Previously, `$input` wasn't very usefull in the `console.command` event, as the input was not yet bound to the command definition.
With this PR, the input is now bound twice: Once before the event is dispatched (to make it usefull in the listeners) and once at the original location in `Command#run()` (to allow changing the input definition in an event listener).

The ``ProgressBarTest`` is passing in 2.7 and 2.8 on Travis only because the ``LegacyProgressHelperTest`` is running first, and so the clock mock is loaded. It would not pass when running it standalone. And the testsuite is currently broken in the master branch because LegacyProgressHelperTest is gone there, and so the clock mock was not loaded before the test.

This PR solves the following issues encountered using question helper and choices questions:
- First choice was not selectable by value.
- ChoiceList with associative choices with mixed string and int keys has same issue with first choice.
- Fix inconsistency by always returning values as strings.
First point exemple:
Last two points are mainly edge cases. Indeed, if a QuestionChoice has something like :

array(
'0' => 'No environment',
'1' => 'My environment 1',
'env_2' => 'My environment 2',
3 => 'My environment 3',
);
```
as choices, you will not be able to select the first choice and get an `InvalidArgumentException`:

For more consistency, the solution is to always return a string.
The issue does not exist in 2.6, as the `QuestionChoice::getDefaultValidator` handled things differently.

- When using the `InputInterface` mock with interactive option, asking a
ChoiceQuestion without specifying max attempts nor a default value led
to an infinite loop when invalid values are submitted. In case there is

This was harder than I thought. To sum up:
* The output now follows the [docopt](http://docopt.org/) specification
* There is a new `addUsage` method to add more usage patterns
* The handling of spaces in the descriptors is refactored to make it easier to understand and to make it render better (using sprintf's features only made it worse imo)

Commands cannot have a double colon in their name (for example, a
command can't be named `foo::bar`). However, one might try to
retrieve a `foo::bar` command from the application like this:
```php
$command = $application->find('foo::bar');
```
The `findAlternatives()` method of the `Application` class fails to
handle these strings when there are commands registered with a name
consisting of at least three parts (e.g. a command is named
`foo:bar:baz`). In this case, an empty string is passed to `strpos()`
causing PHP to raise a warning.

By default, the `ProgressBar` helper overwrites the output to give the nice progress bar look. To prevent the output from blowing up in non-decorated environments, the output was hidden in these environments (see #9846).
This PR enables using the `ProgressBar` in non-decorated environments by adding an `overwrite` flag. When `false`, instead of overwriting the bar, it is rendered on a new line. To prevent flooding the output, you can adjust the `redrawFrequency`.
By default, when using the `ProgressBar` in a non-decorated environment, the `overwrite` flag is set to false. If a `max` is set, the `redrawFrequency` is set to a sensible default (10% of the max). If a `max` isn't set, the bar is output for every advance so to prevent flooding, a sensible `redrawFrequency` should be manually set.
The only BC break is that output will now display where it didn't before.

[Console] Skip commands from ConsoleCommandEvent
Use case: We have different variations of the same application, for which
only certain commands are allowed. Right now this is done in a custom
Application class, but it would be much easier to just be able to skip
commands from a listener, where you can disable commands via the Event
object.
This patch provides this feature and corresponding test cases.

Use case: We have different variations of the same application, for which
only certain commands are allowed. Right now this is done in a custom
Application class, but it would be much easier to just be able to skip

[Console] Added standalone PSR-3 compliant logger
This PR adds a standalone, PSR-3 compliant, logger to the Console component. It logs all messages on the console output. Messages of `DEBUG`, `INFO` and `NOTICE` levels are displayed using the `info` format (default to green). Higher levels are displayed using the `error` formatter (default to red).
This logger is similar to the [Monolog's Console Handler](http://symfony.com/doc/current/cookbook/logging/monolog_console.html) but does not have any external dependency (except `php-fig/log`). This is useful for console applications and commands needing a lightweight PSR-3 compliant logger (e.g. required by a dependency or to display basic informations to the user).

In almost all classes symfony uses property initialization when the value is static. Constructor initialization is only used for things that actually have logic, like passed parameters or dynamic values. IMHO it makes the code much more readable because property definition, phpdoc and default value is in one place. Also one can easily see what the constructor implements for logic like overridden default value of a parent class. Otherwise the real deal is just hidden behind 10 property initializations. One more advantage is that it requires less code. As you can see, the code was almost cut in half (210 additions and 395 deletions).
I unified it accordingly across symfony. Sometimes it was [not even consistent within one class](https://github.com/symfony/symfony/blob/master/src/Symfony/Component/Config/Definition/BaseNode.php#L32). At the same time I recognized some errors like missing parent constructor call, or undefined properties or private properties that are not even used.
I then realized that a few Kernel tests were not passing because they were deeply implementation specific like modifying booted flag with a custom `KernelForTest->setIsBooted();`. I improved and refactored the kernel tests in the __second commit__.
__Third commit__ unifies short ternary operator, e.g. `$foo ?: new Foo()`. __Forth commit__ unifies missing parentheses, e.g. `new Foo()`.

As Composer is now widely used in the PHP world, having to run composer install before running the test suite is expected. This also has the nice benefit of removing a bunch of code, making things easier to maintain (there is only one place to declare a dev dependency), and probably more.

@stof I can't seem to follow the infinite loop you talked about in #8366 . `DialogHelper::ask` will return the default, which is `null`, that breaks the while loop and it returns the default.

The default value for array options will be an array, so it is not suitable to use as the default when processing one of many values for a multi-value option. This is an alternative implementation for adding events to console applications.
This implementation has the following features:
* Available for anyone using the Console component and it is not tied to
FrameworkBundle (this is important as one thing we are trying to solve is
email sending from a command, and frameworks like Silex using the Console
component needs a solution too);
* Non-intrusive as the current code has not been changed (except for renaming
an internal variable that was wrongly named -- so that's not strictly needed
for this PR)
* The new DispatchableApplication class also works without a dispatcher,
falling back to the regular behavior. That makes easy to create applications
that can benefit from a dispatcher when available, but can still work
otherwise.
* Besides the *before* and *after* events, there is also an *exception* event
that is dispatched whenever an exception is thrown.
* Each event is quite powerful and can manipulate the input, the output, but
also the command to be executed.

At the moment it is not possible to overwrite the input arguments of a console application to not have the default ones. Adding is possible with:
$cli->getDefinition()->addOption(new InputOption('--custom', '-c', InputOption::VALUE_NONE, 'Use custom option.'));
Also added some simple tests for adding a custom HelperSet.

@dirkaholic Squashing is about making the PR use only 1 commit instead of 2 (the second one changing only some whitespaces, which is not what its message says). But @fabpot told me that he improved his merging tool and so he can squash it when merging so it is OK.

It adds a method to `DialogHelper` to ask a question and hide the response. It's pretty cool when working with passwords.
This code is more than largely inspired by Composer, see [ConsoleIO.php at line 140](https://github.com/composer/composer/blob/master/src/Composer/IO/ConsoleIO.php#L140)
You will notice that this PR embeds a Windows Executable binary for windows support. This windows binary is provided by @Seldaek (see https://github.com/Seldaek/hidden-input)
This dependency is not yet available via composer.
If this is a problem to embed this file, we can think of other way to provide this support (make a package from HiddenInput and add composer recommandation for example).

And you should also update the changelog.
Btw, adding composer for hiddeninput does not make sense. Compsoer is about installing PHP code, not about downloading the source of a C++ program.

I simplified the change. And the reason why tests for text help do not need changes is because in CommandTest, the commands are executed first which also merges app definition before invoking asText or asXml . While in ApplicationTest, commands are never run therefore app definition is not being merged.
