- When using the `InputInterface` mock with interactive option, asking a
ChoiceQuestion without specifying max attempts nor a default value led
to an infinite loop when invalid values are submitted. In case there is
something wrong in tests, it wasn't obvious to understand what's
happening, as the process was constantly waiting for inputs.
- `testAmbiguousChoiceFromChoicelist` test was wrong as the
autocompleter filled the value and the test was never triggered (+max
attempt issue).
Using question helper and choices questions:
- first choice was not selectable by value.
- ChoiceList with associative choices with mixed string and int keys has same issue with first choice. Add test for all choices.
- Fix inconsistency by always returning values as strings

By default, the `ProgressBar` helper overwrites the output to give the nice progress bar look. To prevent the output from blowing up in non-decorated environments, the output was hidden in these environments (see #9846).
This PR enables using the `ProgressBar` in non-decorated environments by adding an `overwrite` flag. When `false`, instead of overwriting the bar, it is rendered on a new line. To prevent flooding the output, you can adjust the `redrawFrequency`.
By default, when using the `ProgressBar` in a non-decorated environment, the `overwrite` flag is set to false. If a `max` is set, the `redrawFrequency` is set to a sensible default (10% of the max). If a `max` isn't set, the bar is output for every advance so to prevent flooding, a sensible `redrawFrequency` should be manually set.
The only BC break is that output will now display where it didn't before.

[2.3] [Console] TableHelper
When building a console application it may be useful to display tabular data.
`TableHelper` can display table header and rows, customizable alignment of columns, cell padding and colors.
Basic usage example:

$table = $app->getHelperSet()->get('table');
$table
->setHeaders(array('ISBN', 'Title', 'Author'))
->setRows(array(
array('99921-58-10-7', 'Divine Comedy', 'Dante Alighieri'),

It adds a method to `DialogHelper` to ask a question and hide the response. It's pretty cool when working with passwords.
This code is more than largely inspired by Composer, see [ConsoleIO.php at line 140](https://github.com/composer/composer/blob/master/src/Composer/IO/ConsoleIO.php#L140)
You will notice that this PR embeds a Windows Executable binary for windows support. This windows binary is provided by @Seldaek (see https://github.com/Seldaek/hidden-input)
This dependency is not yet available via composer.
If this is a problem to embed this file, we can think of other way to provide this support (make a package from HiddenInput and add composer recommandation for example).

The link to the hiddeninput source code should be added in the readme.
And you should also update the changelog.
Btw, adding composer for hiddeninput does not make sense. Compsoer is about installing PHP code, not about downloading the source of a C++ program.
