139f591458 fix catch and throw: make sure we have an environment. 
use $notrace consistently: note that it only matters if ran from the
debugger, otherwise it is just a meta-call.

4c933b8d79 implement thread_cancel using signals 
use duplicate_term instead of copy_term in throw: throw may lose
reference to term.
