d3f969e1b1 various_codes includes all the head pointers to the code space. Try to 
centralise everything in a single file (HEAPFIELDS) and generate them
from a Prolog script (buildheap).

b72345cc97 handle correctly throws within the scope of a call_cleanup. This is implemented 
by using an extra argument to verify if the catcher is the current call_cleanup.
